WHS Incident Reporting
After a recent incident in the school, it was found that not all school members were aware of the requirements for incident reporting timeframes and incident site preservation.

Incident Reporting Timeframes
Incidents must be reported in Figtree within 48 hours of the occurrence. Please note you can report an incident on behalf of another person. If you are unsure whether an incident has already been reported or need assistance with the process, please contact the WHS Office.

Incident Site Preservation
After an incident site has been made safe it must be preserved for inspection by the investigation team. This can be done by placing barriers around the site or by closing the area. The site is not to be touched other than to make it safe until it has been released by the lead investigator. If you need assistance with securing a site, contact the WHS office.

Training
Information about incident reporting and site preservation can be found in 2 training courses on Pulse:

(Use search function within pulse to find course)

Both courses are mandatory for all workers in the school. If you have not completed these courses or if you need a refresh on the information in this notice, please complete asap.

The Figtree training course must be completed at least every 3 years. If it has been longer since you have completed it, please refresh the course.

If you have any questions regarding incident reporting or the training course, please contact the WHS office.
